Deploy pipeline

Three gates enforce that what ships is what a reader sees. In the 2026-08-07/08 incidents, two failure modes surfaced: internal-marked documents accidentally published, and fixes that were in HEAD still serving pre-fix content at the canonical URL. Gate 1 blocks the first; Gates 2+3 close the second.
